Coaching Agreement

A clear understanding builds the foundation for effective coaching. Our Life Coaching Agreement outlines the expectations, confidentiality, and responsibilities shared between you and your coach. It ensures transparency, professionalism, and mutual respect as you begin your coaching journey.

At Heart & Mind Counseling Services, we believe that a written agreement helps establish trust, clarify goals, and create a supportive structure for lasting personal and professional growth.

This coaching agreement is made between the client and Professional Life Coach, Christopher Dietz, to be referred to as the "Coach". By signing this agreement, the client agrees to the following:

  1. Coaching is not the same as mentoring. The Coach will not provide guidance or advice based on prior life or work experience.

  2. The coaching relationship is in no way to be construed as psychotherapy, psychological counseling, or any type of therapy. If the Client feels the need for professional counseling or therapy, it is the responsibility of the Client to seek a licensed professional. The Practice Manager can recommend a therapist on staff upon request.

  3. The number of coaching sessions is determined by the Client.

  4. Coaching sessions will take place virtually.

The Client and Coach agree to enter into a coaching relationship pursuant to the terms

stated below:

 

The Coach’s Role

Your coach's primary responsibilities are to:

  • Help you maintain your primary focus.

  • Be fully present during each coaching call/meeting.

  • Provoke your learning and action by asking questions.

  • Share his intuition with you.

  • Reflect on both your spoken and unspoken messages.

  • Hold himself accountable.

  • Be on time for the coaching call/meeting.

  • Help you determine when you may need professional therapy versus when you need coaching.

  • Remain available for 10 minutes past the agreed-upon time, and end on time.

 

The Client's Role

As the Client, you agree to:

  • Identify your primary focus area(s), goals and objectives.

  • Commit to learn more about and act on your focus area(s).

  • Help the coach determine when you may need professional therapy versus when you need coaching.

  • Establish the topic, goals, objectives, and outcome for each coaching session.

  • Be candid and courageous with yourself and your coach.

  • Stretch and challenge yourself, and enjoy the process.

  • Hold yourself accountable.

  • Be on time for the coaching call/meeting.

Disclosure of Information

These sessions do not benefit from the privilege associated with attorney/client, doctor/patient, chaplain/penitent, or therapist/counselee relationships. Your Coach will only share and disclose the content of the coaching sessions in these situations:

  • Supervision and internal team meetings at Heart and Mind Counseling Services

  • You provide, in writing, authorization of the disclosure of the information outside of my supervisor and team at Heart and Mind Counseling Services

  • You report an act of fraud, waste, or abuse.

  • You reveal that you have committed or intend to commit a crime or harm yourself or others.

  • You share information that violates security clearance requirements.

  • You share information regarding sexual harassment; and/or

  • Your coach is required by applicable laws and/or court order to share particular information.

 

Permission to use Client’s Name and Contact Information:

As part of the certification process for the International Coach Federation (ICF) and other certification boards, a coach seeking credentialing may be required to provide client names and contact information. In the event the coach seeks certification by ICF or another certification board, the client gives the coach permission to disclose his/her name and contact information for credentialing purposes to the certification board. In addition, the client gives the coach permission to audio or video record all sessions for training and credentialing purposes. Recordings will be shared with the client upon his/her request.

Expectations

Coaching is not available to address issues associated with sexual harassment. If you raise a sexual harassment issue, your Coach will ask you to contact the appropriate authorities in your workplace or law enforcement. Coaching is not to be used as a substitute for professional advice by legal, medical, financial, business, spiritual, or other qualified professionals. If the client is under the care of a mental health professional, they must consult with the mental health care provider regarding the advisability of working with a coach and make that health care provider aware of the decision to proceed with the coaching relationship. The client is fully responsible for his/her physical, mental, and emotional well-being during coaching calls, including his/her choices and decisions. At the request of either party, the coaching may be discontinued at any time.

Scheduling

Cancellations should be communicated at least 24 hours in advance. The cancelling party agrees to make a reasonable effort to reschedule a missed call/meeting within five business days. Clients may be subject to a no-show fee.
